1) I agree to check in regularly with and pray for my mentee, attend mentor-mentee functions, share activities every month or two, and occasionally attend school events.
2) I agree to observe and abide by the Safety Covenant and the guidelines regarding working with children and youth.
3) I agree to consult with one of the pastors if I have any concerns about the safety of my mentee.
4) I have not been charged with any crime involving abuse, neglect, or violence.
